Help Members Take Charge of Their Health Alignment Health is seeking a compassionate and motivated Bilingual Spanish Health Coach & Educator to support members through in-home, telehealth, and telephonic visits across Los Angeles and Orange County. In this rewarding role, you'll build meaningful relationships with members, caregivers, and families while providing health coaching, education, encouragement, and access to resources that help improve overall health and well-being. Candidates must be comfortable conducting home visits throughout Los Angeles and Orange County, California.
Responsibilities
- Conduct home and virtual visits with Alignment Health members.
- Provide wellness coaching to help reduce high-risk behaviors and support healthy lifestyle changes.
- Collaborate with Advanced Practice Clinicians and Social Workers to create personalized action plans.
