Help Members Live Healthier Lives Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout Maricopa County, Arizona. In this highly field-based role, you'll build trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits, access community resources, and improve their overall well-being. Applicants must be comfortable conducting daily in-home visits and traveling throughout Maricopa County, Arizona.

Responsibilities

• Conduct approximately 6-8 in-home visits daily throughout your assigned service area.
• Provide wellness coaching and health education to support behavior change and healthier lifestyles.
• Partner with Advanced Practice Clinicians and Social Workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction.
• Coordinate care with internal clinical teams and external partners.
• Deliver health education materials to members, caregivers, and families.

Requirements

• Minimum 1+ year of experience as a wellness health coach or educator.
• Current, valid, unrestricted driver's license.
• Reliable personal transportation and ability to travel throughout the assigned territory.
• Ability to maintain automobile insurance in accordance with company requirements.
• Ability to work independently and remotely.
• Strong communication, organizational, time management, and relationship-building skills.
• Proficiency with Microsoft Office (Excel, Word
