About the Role
The Field Based Community Health Worker (CHW) acts as a liaison with Medicaid members, providing home and social assessments, member education, and support to access appropriate care. They engage primarily face-to-face in the community, building positive relationships to promote health management, social support, and participation in health programs.
Responsibilities
- Conduct outreach to Medicaid members
- Facilitate provider follow-up
- Assist with community resource navigation
- Support members in engaging in work or volunteer activities
- Manage multiple tasks, work independently and in teams
- Adapt to diverse community settings
Requirements
- High school diploma or higher
- 1+ year of community resource experience
- Bilingual in Spanish and English
- Reliable transportation
- Proficiency in MS Word and Outlook
Preferred Qualifications
- Healthcare certifications
- Experience working with behavioral health, substance use disorders, or managed care
Schedule
Flexible hours including some evenings and weekends. Travel required (50-75%).
