About the Role
The Field Based Community Health Worker (CHW) acts as a liaison with Medicaid members, providing home and social assessments, member education, and support to access appropriate care. They primarily engage face-to-face or via phone, fostering positive relationships, encouraging healthcare participation, and connecting members with community resources. The role involves community outreach, managing multiple tasks, and understanding diverse community cultures.
Flexibility to work in community settings and some evenings/weekends is required.

Requirements
- High school diploma or GED
