Bereavement Coordinator

Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

ESSENTIAL FUNCTIONS

  • Provide bereavement support to families through maintaining and monitoring the designated telephonic bereavement line, supportive phone calls, short-term grief counseling, referrals to additional resources, memorial services, support groups, and other Grief Care activities.
  • Assess each family member and develop an individualized plan of care for the bereaved in conjunction with the hospice team.
  • Create a calendar for Grief Care activities for the year.
  • Plan and implement new programs for the hospice bereaved and the community.
  • Work with Volunteer Coordinator in training bereavement volunteers in knowledge base and skills to provide meaningful support to families.
  • Assist in the development of collaborative opportunities for volunteers, internships, and professional volunteers.
  • Oversee all processes for the Grief Care program, including mailings, IDG preparation, and call scheduling and documentation.
  • Provide debriefing and emotional support for hospice staff.
  • Affirm that a high standard of bereavement care is maintained through satisfaction surveys and audits.

QUALIFICATIONS

  • Degree in Social Work, Counseling, or related field. Master’s Degree Preferred.
  • 2 years of grief-related work experience preferred.
  • Proficient skill using a computer is required; Microsoft office suite experience required.
  • Must have reliable transportation.
  • Must be sensitive to the needs of bereaved survivors and aggressive in developing and providing appropriate services.
