About the role
This position involves planning, implementing, and evaluating therapy for assigned patients. Social work services are also provided, including discharge planning and linkage to community resources.
Responsibilities
- Assesses clients for personal, social, and emotional development, family dynamics, and biopsychosocial factors.
- Participates in multidisciplinary treatment planning, considering special treatment needs due to age, gender, sexual orientation, or ethnic origin.
- Affordably assesses patients for discharge planning needs based on their ability to make decisions and manage their care.
- Knowledgeable of community resources and eligibility criteria for accessing them.
- Petitions for final commitments and longer-term psychiatric care when necessary.
- Interprets social, psychological, and emotional problems and shares insights with the treatment team.
- Assesses patients for signs of abuse and coordinates referrals and counseling.
- Documents services provided in the medical record, including evaluations and treatment plans.
- Facilitates individual, group, and family counseling using evidence-based practices.
- Communicates with patients, families, and referral resources, discussing topics like advance directives and patient rights.
- Creates therapeutic activities and implements them during scheduled groups.
- Follows up with patients post-discharge and documents discharge instructions.
- Maintains and monitors staff compliance with hospital and departmental policies and procedures for safety, security, and infection control.
- Manages time effectively to balance patient treatment needs, documentation, and educational requirements.
- Communicates and interacts professionally with patients, families, visitors, physicians, and other healthcare personnel.
- Remains current in healthcare delivery and professional literature, participating in quality initiatives and continuing education.
Requirements
Must possess current licensure as required by state board where services will be provided. Education, certification, and/or licensure requirements vary by state.
Qualifications
Two (2) years training and/or experience in Behavioral Health Therapy is preferred.
Skills
- Knowledge of community resources and criteria for accessing them.
- Ability to interpret social, psychological, and emotional problems.
- Proficiency in evidence-based practices for counseling.
- Effective communication and interpersonal skills.
- Good organizational and time management skills.
- Ability to manage multiple tasks and prioritize responsibilities.
- Knowledge of relevant laws and regulations regarding abuse, neglect, and domestic violence.
