Behavioral Health Provider and Community Relations – LCSW or LMHC-D - Full Time

Position Summary

The Behavioral Health Provider serves as the primary liaison between the organization, healthcare providers, and community partners to expand access to behavioral health and substance use disorder services. This position is responsible for developing and strengthening referral networks, representing the organization at community outreach events and professional forums, providing behavioral health consultation to internal and external stakeholders, and fostering collaborative partnerships that improve care coordination and patient access. The role supports the organization's strategic objectives by increasing community awareness, enhancing referral pathways, promoting integrated behavioral healthcare, and contributing to the growth and sustainability of inpatient and outpatient behavioral health programs.

Responsibilities

• Develop, cultivate, and maintain referral relationships with primary care providers, hospitals, behavioral health programs, and community-based organizations to increase awareness and utilization of the organization's inpatient and outpatient behavioral health and substance use disorder services.
• Represent the organization at community events, health fairs, professional meetings, coalitions, and community task forces to promote behavioral health services and strengthen the organization's presence within the community.
• Serve as a liaison between the hospital, healthcare providers, community agencies, and social service organizations to improve care coordination, facilitate access to services, and support continuity of care.
• Educate physicians, referral sources, community partners, and healthcare professionals regarding available behavioral health programs, admission criteria, and referral processes.
• Identify barriers to patient referrals and access to care and develop strategies to improve referral pathways, program utilization, and patient engagement.
• Utilize advanced behavioral health assessment and clinical expertise to provide consultation, promote early identification of behavioral health needs, and collaborate with interdisciplinary healthcare providers to facilitate appropriate referrals and treatment planning.
• Establish and maintain collaborative partnerships with community organizations, government agencies, and other stakeholders to expand access to behavioral health resources and improve patient outcomes.
• Monitor referral trends, outreach activities, and community needs to identify opportunities for program development, service expansion, and strategic growth.
• Participate in organizational committees, community initiatives, and interdisciplinary teams that support behavioral health integration, community outreach, and population health objectives.
• Support organizational strategic initiatives by increasing community awareness, strengthening referral networks, enhancing patient access to services, and contributing to the long-term growth and sustainability of behavioral health programs.
• Maintain current knowledge of behavioral health resources, community services, regulatory requirements, and industry best practices to effectively support patients, providers, and organizational initiatives.
• Perform other related duties as assigned in support of departmental and organizational goals.

Required Qualifications

• Master's Degree in Social Work or equivalent Mental Health Master's Degree.
• Licensed Clinical Social Worker (LCSW), or Licensed Mental Health Counselor with Diagnostic Privilege (LMHC-D).

Preferred Qualifications

• Credentialed in Alcohol and Substance Abuse (CASAC).

Physical Demands

While performing the duties of this job, the employee is regularly required to communicate with patients, office staff and medical providers. The employee must be able to exchange accurate information in these situations. Ability to travel. Must be able to communicate orally.

Pay

$76,000 - $100,000
