About the Role
Responsible for supporting young adults experiencing their first mental episodes within a community health setting.
Responsibilities
- Assess patient preferences, barriers, and needs, including cultural and emotional factors.
- Develop, implement, and document individualized care plans in collaboration with the interprofessional team, patients, and caregivers.
- Promote respectful, equitable, and empathetic interactions with diverse populations, supporting holistic care.
- Monitor patient condition changes, communicate updates to the team and family, and update care plans accordingly.
- Ensure evidence-based practice and quality outcomes through ongoing evaluation of care.
Requirements
- RN license with 2-5 years of experience.
- Comfortable transporting clients and working both in clinic and community settings.
- Ability to work alertly and safely in a safety-sensitive role.
Preferred Qualifications
- Bachelor's degree in Nursing.
