About the Role

Responsible for supporting young adults experiencing their first mental episodes within a community health setting.

Responsibilities

  • Assess patient preferences, barriers, and needs, including cultural and emotional factors.
  • Develop, implement, and document individualized care plans in collaboration with the interprofessional team, patients, and caregivers.
  • Promote respectful, equitable, and empathetic interactions with diverse populations, supporting holistic care.
  • Monitor patient condition changes, communicate updates to the team and family, and update care plans accordingly.
  • Ensure evidence-based practice and quality outcomes through ongoing evaluation of care.

Requirements

  • RN license with 2-5 years of experience.
  • Comfortable transporting clients and working both in clinic and community settings.
  • Ability to work alertly and safely in a safety-sensitive role.

Preferred Qualifications

  • Bachelor's degree in Nursing.
