About the role
The BH Program Navigator is a member of the Integrated Behavioral Health team and works in continuous partnership with the patient, their family when appropriate, IBH providers, CommuniCare+OLE care team, and relevant community partners to improve health outcomes and meet the patient's desired health and wellness goals.
Responsibilities
- Processes and closes the loop on internal and external behavioral health referrals.
- Supports patient access to behavioral health care and real time screening for virtual appointments.
- Serves as the point of contact, advocate, and informational resource for patients, family/caregiver(s), primary care team, payers, and community resources.
- Facilitates patient access to appropriate behavioral health providers and services, both internal and external.
- Contacts scheduled initial IBH appointments to assess for readiness for appointment.
- Minimizes missed opportunities by managing the IBH schedules and patient wait list.
- Develops and maintains a relationship with community partners to ensure robust collaboration.
- Participates in huddles, team meetings, and case conferencing as needed.
- Helps facilitate warm handoffs at all sites.
- Collaborates with care coordinators to limit overlap and ensure closed-loop IBH referrals.
- In Napa County, Coordinates workflow for Napa County MH Access co-located clinician and CCOLE patient.
- Works as back-up for IBH Program Navigator and Psychiatric MA.
- Documents timely and efficiently in the EMR.
- Determines the appropriate level of IBH services needed to support the patient at any given point in time.
- Works as back-up for other IBH Program Navigators and Psychiatric MAs.
- Attends appropriate training courses/webinars and meetings.
- Gathers and reports data related to behavioral health when needed.
- Provides feedback for the improvement of Integrated Behavioral Health processes.
- Performs other duties as assigned.
Requirements
- Education: High School Diploma or equivalent required. Undergraduate degree in social services preferred.
- Experience/Lived Experience: Experience working with diverse populations displaying respect and sensitivity to diversity, equity and inclusion preferred. Experience working in a community health setting preferred. Experience working with complex patient situations a plus.
- Special Skills/Training: Strong verbal and written communication skills required; Bilingual (English/Spanish) required. Strong collaborative skills: ability to effectively and appropriately engage members of the care team, other clinic departments, leadership, and external community partners. Exceptional communication skills; ability to effectively engage patients and assess needs and concerns to ensure that the patient is referred appropriately. Excellent in-person and phone customer service skills; able to manage difficult and emotional patient situations and to respond promptly to patient needs. Ability to adapt to change; approach challenges with an innovative and proactive attitude; and investigate resources. BLS Certification Required.
