Behavioral Health Navigator

About the Role

Progressive Community Health Centers is seeking an experienced professional to serve as a Behavioral Health Navigator at our Lisbon Avenue Health Centers. This role involves implementing care plans designed by social workers to address patients' identified needs, resources, and goals. The position promotes patient self-management and independence, requires maintaining accurate documentation, and fosters collaborative teamwork while adhering to organizational policies and ethical standards.

Responsibilities

• Providing coordinated, patient-centered support that ensures timely, effective, and seamless delivery of care.
• Identifying and effectively utilizing community resources to meet the needs of patients/families.
• Participating in departmental and organizational committees as applicable.
• Implementing the plan developed by the Social Worker that addresses the patient's identified needs, issues, resources, and care goals.
• Educating patients/families on available options and prioritizing patients according to intensity, need, and required follow-up.
• Promoting patient self-management and empowering patients and families to achieve maximum wellness and independence.
• Assisting patients with needed items, including transportation, scheduling, housing, food sources, etc.
• Maintaining accurate and timely documentation.
• Adhering to organizational policies and procedures, including federal/state regulations related to patient confidentiality, release of information, and HIPAA.

Qualifications

• A bachelor's degree in social work, human services, psychology, public health, criminal justice, sociology, anthropology, or related field.
• Experience preferred within Milwaukee County community resource settings.

Skills

• Strong communication skills and cultural competency.
• Ability to work independently and as part of a team.
• Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA.

Schedule

Monday - Friday, 8 a.m. - 5 p.m. No late nights, holidays, or weekends required.

We provide high-quality, comprehensive care to approximately 16,000 patients annually, mostly low-income individuals. Our services include family medicine, internal medicine, women's health, urgent care, dental, radiology, and integrated behavioral health, as well as supportive services like case management, health education, and benefits enrollment.
