About the role
The Behavioral Health Case Manager – Missouri role is field-based, enabling associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training.
Responsibilities
- Uses appropriate screening criteria knowledge and clinical judgment to assess member needs.
- Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
- Makes regular evaluations of the effectiveness of the care plan and modifies the plan as necessary.
- Serves as a liaison between members and various internal and external resources to ensure access to appropriate quality and cost-effective care.
- Coordinates with providers to meet the identified needs of the members.
Requirements
- Requires a Master’s Degree in Social Work, Counseling, or a related behavioral health field, or a degree in Nursing, and minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
- A current active unrestricted license such as RN, LCSW (as applicable by state law and scope of practice), LMHC, LPC (as allowed by applicable state laws), LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States is required.
Preferred qualifications, skills, and experiences
- Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred.
- Candidates residing in Columbia or Springfield are encouraged to apply. Candidates residing in the Kansas City, MO area can also be considered.
- Experience with community mental health and outpatient services is preferred.
