Behavioral Health Case Manager I

Shift: Monday – Friday, 9:00am – 5:00pm. This role is primarily virtual with about 20% field-based work. Candidate must be within the state of Virginia and licensed in VA. Alternate locations within Virginia may be considered if candidates reside within commuting distance from an office.

About the Role

The Behavioral Health Case Manager I performs case management telephonically and/or via home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.

Responsibilities

• Uses appropriate screening criteria, knowledge, and clinical judgment to assess member needs.
• Conducts assessments to identify individual needs and develops care plans to address objectives and goals as identified during assessment.
• Monitors and evaluates the effectiveness of care plans and modifies plans as needed.
• Supports member access to appropriate quality and cost-effective care.
• Coordinates with internal and external resources to meet identified needs of members and collaborates with providers.

Requirements

• MA/MS in social work, counseling, or a related behavioral health field, or a degree in nursing, and a minimum of 3 years of clinical experience in social work counseling with a broad range of experience in complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
• Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW (as allowed by applicable state laws), or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.
• For Government business only, LAPC or LAMFT (as allowed by applicable state laws) is also acceptable in addition to other licensure referenced above; and any other state or federal requirements that may apply.
• For associates working within Puerto Rico who are member or patient-facing either in a clinical setting or in the Best Transportation unit, a current PR health certificate and a current PR Law 300 certificate are required for this position.

Preferred Skills

• Experience in case management and telephonic and/or in-person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders.

Pay

For candidates working in person or virtually in Virginia, the salary range for this position is $61,028.00 to $96,360.00. The salary offered is based on a number of legitimate, non-discriminatory factors set by the Company.

Schedule

This role follows a hybrid workforce strategy. Associates are required to work at an Elevance Health location at least once per week, with specific requirements discussed during the hiring process. The role is primarily virtual, with required in-person training sessions for onboarding and skill development.
