Behavioral Health Case Manager I

About the Role

The Behavioral Health Case Manager is responsible for managing an assigned caseload of clients and assessing client needs by developing, implementing, and reviewing service plans, and working with other internal and external resources to meet and achieve client service needs.

Qualifications

• High school diploma or GED from an accredited institution is required; Associate’s Degree preferred
• Minimum of one (1) year of experience working in the Behavioral Health field preferred
• Excellent customer service and computer skills
• Thorough knowledge of current versions of DSM and ICD preferred
• Ability to complete a Behavioral Health Assessment with appropriate clinical oversight
• Ability to provide effective and respectful quality care and services responsive to diverse cultural beliefs and practices
• Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses following American Heart Association or Red Cross guidelines
• Bilingual English/Spanish strongly preferred
• Valid Arizona driver’s license
• Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety

Responsibilities

• Participate in screening new service requests to ensure individuals are adequately and appropriately served according to policies and procedures
• Attend regular interdisciplinary staff meetings
• Accept newly assigned cases where diagnostic intake has been completed by the Intake Specialist; complete case management assessments for all clients entering caseload
• Develop appropriate treatment/service plans with clients to implement services and build effective alliances
• Provide direct client services, including case management, skills training, group skills training, health promotion, transportation, supportive employment services, and counseling with appropriate clinical oversight; services may be provided in office, home, school, or community settings
• Provide ongoing supportive and/or case management functions aligned with the service plan to assist clients in achieving goals and objectives
• Conduct face-to-face reviews with clients and/or relevant behavioral health staff regularly; report progress to modify service plans as needed
• Assist supervisory staff with client tracking, including generating reports to ensure quality of care follow-through
• Document all service contacts, including interviews, collateral contacts, and correspondence, maintaining case records per internal and regulatory standards
• Maintain close communication with consulting psychiatrist and Behavioral Health team regarding medication compliance, side effects, and changes; alert the team to any client adjustment changes suggesting decompensation
• Perform other duties as assigned by the supervisor, consistent with Adelante Health Care policies and procedures

Requires close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
• Work environment: Exposure to weather conditions when traveling to various clinic sites. Moderate noise level. Potential exposure to health hazards (contagious diseases, blood-borne pathogens) when working in clinic areas.
