LOCATION: This is a virtual eligible role. Indiana residency is required. HOURS: General business hours, Monday through Friday. This role enables associates to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.

About the role

The Behavioral Health Case Manager I is responsible for performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral and mental health needs. Uses appropriate screening criteria knowledge and clinical judgment to assess member needs. Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment. Monitors and evaluates effectiveness of care plan and modifies plan as needed. Supports member access to appropriate quality and cost effective care. Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.

Responsibilities

  • Uses appropriate screening criteria knowledge and clinical judgment to assess member needs.
  • Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
  • Maintains and updates care plan as needed.
  • Evaluates effectiveness of care plan and modifies plan as needed.
  • Serves as a resource for members regarding available services and resources.
  • Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.
  • Supports member access to appropriate quality and cost effective care.
  • Collaborates with providers to ensure continuity of care.
  • Monitors and evaluates the progress of members and adjusts care plans accordingly.
  • Provides support and guidance to members to help them navigate the healthcare system.
  • Ensures compliance with regulatory standards and policies.
  • Participates in regular meetings with supervisors and colleagues to discuss case management activities and challenges.
  • Assists in the development and implementation of training programs for new case managers.

Requirements

  • Requires MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background.
  • Requires a current active unrestricted license as an LCSW, LMHC, or RN with psychiatric mental health certification from the State of Indiana.
  • Substance abuse crisis experience is strongly preferred.
  • Medicaid experience is helpful for this position.
  • Experience in case management and coaching with members with a broad range of complex substance abuse/psychiatric disorders is preferred.

Qualifications

  • Master's degree in Social Work, Counseling, or a related behavioral health field.
  • Minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment.
  • Current active unrestricted license as an LCSW, LMHC, or RN with psychiatric mental health certification from the State of Indiana.
  • Substance abuse crisis experience is strongly preferred.
  • Medicaid experience is helpful for this position.
  • Experience in case management and coaching with members with a broad range of complex substance abuse/psychiatric disorders is preferred.

Skills

  • Strong clinical judgment and assessment skills.
  • Effective communication and interpersonal skills.
  • Ability to work independently and manage multiple tasks simultaneously.
  • Knowledge of behavioral health and substance abuse disorders.
  • Ability to coordinate with various healthcare professionals and community resources.
  • Proficiency in Microsoft Office Suite.
