Responsibilities
- Engaging and collaborating with facility staff, including Executive Directors, Directors of Nursing, case managers, social workers, staff developers, admissions personnel, surveyors, and physicians.
- Facilitating the coordination of a variety of services, including Psychiatry, Therapy, and Neuropsychology (where available), to meet the needs of patients in the facilities.
- Aiding facilities in processing referrals and identifying residents who could benefit from psychiatric and therapy services.
- Coordinating the completion of consents and providing assistance when needed.
- Educating facility staff on Behavioral Health Solutions processes, addressing questions, and resolving concerns.
- Attending behavior management meetings with facility staff and coordinating MD attendance for facility meetings such as GDR and QAPI.
- Ensuring compliance with PASRR II requirements by working closely with both BHS and the facility.
- Scheduling in-services and educational sessions for facility staff.
- Facilitating Telehealth visits with clinicians as needed to enhance patient care.
- Collaborating with the Business Development team to support the facility onboarding process.
Qualifications
- An individual must be able to perform each essential duty satisfactorily.
- A minimum of a High School Diploma or GED, with some college education focused on behavioral health or psychology preferred.
- At least 3-5 years of experience working in skilled nursing facilities as a Licensed Social Worker, Support Staff, or Clinical Care Coordinator.
- Proficiency in using a PC, Windows environment, the Internet, and MS Office Applications such as Outlook, Word, and Excel.
- Experience with EHR software systems (Athena) is preferred.
Skills
- Familiarity with the survey process in skilled nursing facilities.
- Knowledge of behavioral/medical terminology, therapy, pharmacology, and related procedures.
- Strong organizational skills, including strong attention to detail, data-driven mindset, and proficiency in utilizing technology.
- Ability to multitask and thrive in a fast-paced environment with evolving opportunities.
- Effective critical thinking and problem-solving skills to address challenges in patient care coordination.
- Strong ability to build and maintain positive relationships with facilities, providers, and the supervisory team.
- Excellent written and verbal communication skills to interact effectively with various stakeholders.
