About the role
Community Bridges is seeking a Full-time Behavioral Direct Support Professional in Concord, NH.
Responsibilities
- Aid individuals with planned activities and community involvement.
- Develop a relationship map to gather information about the individual to establish social inclusion opportunities.
- Assist individuals in developing a network of people in their lives.
- Support individuals in acquiring daily functional living skills.
- Represent Community Choices and its services to the community with a positive and professional image.
- Perform emergency behavioral intervention following behavioral intervention techniques and individual behavioral plans.
Requirements
- High School Diploma or GED.
- Experience in the human service or related field with persons experiencing disabilities is desirable.
- MOAB Day 1, MANDT, or formal de-escalation training.
- Reliable transportation, a valid driver's license, and proof of automobile insurance.

- Ability to communicate thoughts and ideas well in both verbal and written formats.
- Strong personal boundaries.
- Ability to meet required deadlines for documentation of care.
- Ability to work independently and demonstrate sound judgment and decision-making skills.
- Ability to assist individuals to acquire daily functional living skills.
- Ability to obtain information about and utilize community resources, activities, and social opportunities relevant to supported individuals.
- Ability to represent Community Choices and its services to the community with a positive and professional image.
- Ability to work as a member of a team and carry out team decisions.
- Ability to demonstrate computer competencies: utilize email and word processing specifically.
- Ability to demonstrate a positive attitude under stressful situations with individuals, co-workers, and people external to the organization.
