Responsibilities

• Provide high-quality home healthcare to adult patients with complex needs, including trach/vent care and mobility support using Hoyer lifts.
• Ensure clinical outcomes through compassionate patient care, documentation via electronic tablets, and collaboration with team members.
• Maintain compliance with licensing, certifications, and health requirements.

Qualifications

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification with hands-on component
• TB test within the last 12 months
• Reliable transportation
• Preferred: Six months of hands-on nursing experience
