Responsibilities

• Provide skilled nursing care in patients' homes.
• Deliver compassionate, patient-centered care in clients' homes.
• Utilize electronic documentation via tablets in patient homes.
• Document care electronically.
• Support patient health and safety through compassionate intervention.
• Collaborate with a multidisciplinary team to achieve optimal clinical outcomes.

Requirements

• Active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification with hands-on component (training support available if needed).
• TB skin test within the last 12 months.
• Reliable transportation.

Qualifications

Preferred: Six months of hands-on nursing experience.
