Responsibilities

• Deliver compassionate nursing care in clients’ homes.
• Document patient progress electronically using provided tablets.
• Collaborate with a multidisciplinary team to ensure optimal clinical outcomes.
• Adhere to safety, health, and documentation standards.

Qualifications

• Active, unencumbered LPN/LVN license in the practice state.
• Current CPR certification with hands-on component.
• TB skin test within the last 12 months.
• Reliable transportation required.
• Prior nursing experience preferred but not mandatory.
