About the role

Responsibilities
- Deliver compassionate care to patients in their homes.
- Document patient care electronically using tablets provided in each patient's home.
- Work collaboratively with other healthcare professionals to ensure high-quality clinical outcomes.
- Adhere to all health-related requirements, including required vaccinations and screenings.
Requirements
- Maintain an active, unencumbered license (LPN/LVN) in the state of practice.
- Current CPR certification with hands-on component.
- TB skin test (within the last 12 months).
- Six months of hands-on nursing experience preferred.
Qualifications
- Reliable transportation.
Skills
Strong communication skills, ability to work independently, and a compassionate nature are essential.
