We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Assisted Living team. ✔ Supportive, team-oriented environment ✔ Build meaningful relationships with residents and families ✔ Opportunity to develop senior living and clinical leadership experience ✔ Leadership that values and supports staff ✔ Meaningful work where you can make a difference every shift Position Overview The LPN/RN Nurse provides nursing support and oversight for residents of The Terraces while promoting independence, dignity, choice, safety, and quality of life. The Nurse is responsible for monitoring resident health and wellness, identifying and communicating changes in condition, supporting medication management, coordinating with providers and families, completing required documentation, and providing clinical guidance to direct-care team members. This position works closely with facility leadership, Medication Aides, CNAs, providers, residents, families, and other members of the care team to ensure resident needs are appropriately addressed. Key Responsibilities Resident Care & Wellness Monitor the health, safety, and overall wellness of residentsComplete nursing assessments and evaluations as assignedObserve residents for changes in physical, cognitive, behavioral, or emotional conditionObtain and monitor vital signs and other clinical information as appropriateProvide nursing care and treatments within applicable scope of practiceSupport residents in maintaining the highest practical level of independenceRespond appropriately to resident emergencies and urgent clinical concerns Medication Management Administer medications and treatments within applicable nursing scope of practiceMonitor medication administration and resident response to medicationsSupport safe medication-management practicesProvide guidance to Medication Aides within applicable scope and facility proceduresEnsure medication-related documentation is accurate and completed timelyCommunicate medication concerns to providers and leadership as appropriate Changes in Condition & Care Coordination Recognize and respond to changes in resident conditionCommunicate significant changes promptly to the appropriate providerCoordinate with families or responsible parties as appropriateReview new orders and ensure appropriate communication and implementationHelp ensure continuity of care between residents, providers, families, and facility staff Resident Service Plans & Documentation Participate in resident assessments and individualized service planningHelp ensure services reflect resident needs, preferences, abilities, and identified risksComplete nursing documentation accurately and within established timeframesDocument assessments, changes in condition, provider communication, treatments, and other nursing interventionsSupport appropriate documentation by direct-care team members Clinical Leadership & Team Support Serve as a clinical resource for CNAs, Medication Aides, and other care team membersProvide direction and delegation within applicable scope of practice and facility proceduresAssist staff with resident-care questions and clinical concernsPromote accountability and timely communication of resident changesSupport new employee orientation and ongoing staff education as assigned Resident & Family Experience Build positive and professional relationships with residents and familiesCommunicate with residents in a compassionate and respectful mannerRespond professionally to resident and family questions or concernsEscalate unresolved concerns to appropriate leadershipMaintain resident privacy, confidentiality, dignity, and choice Safety & Infection Prevention Follow established infection-prevention and control practicesPerform appropriate hand hygiene and use personal protective equipment when requiredMonitor for potential infection or illness and communicate concerns appropriatelySupport fall-prevention and resident-safety interventionsIdentify and report environmental or safety hazardsRespond appropriately to accidents, incidents, injuries, and emergenciesParticipate in emergency preparedness and facility emergency proceduresPromote safe care practices throughout the community Qualifications Active Kansas Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standingAbility to communicate effectively with residents, families, providers, and team membersAssisted living, senior living, long-term care, skilled nursing, rehabilitation, or related healthcare experience preferredExperience working with older adults preferredCommitment to resident dignity, independence, safety, and quality of life Benefits Competitive compensation401(k) retirement planHealth, dental, and vision insurance for eligible employeesPaid Time Off (PTO)Care Cash ProgramEmployee Referral Bonus ProgramSupportive and collaborative work environmentTraining and professional development opportunitiesOpportunities for career advancement Grow With Us The Nurse position at The Terraces provides an opportunity to develop experience in assisted living, resident assessment, medication management, care coordination, clinical leadership, and senior living operations.
