Assistant Habilitation Supervisor

Qualifications

  • Associate Degree and one-year experience working with the developmentally disabled preferred; or
  • High school diploma or equivalency and two years’ experience working with the developmentally disabled required.
  • Valid/acceptable NYS driver’s license.
  • Authorized to work in the United States.

Summary

Serve as the frontline supervisor to Direct Support Professional staff working in assigned program area. Provides daily supervision and is responsible for the daily operation of the day program site, working under the direction and guidance of the Habilitation Program Coordinator. Spend a majority of working hours in direct contact and interaction with Persons We Support. Ensure timely and thorough completion of all paperwork and Therap documentation for Persons We Support in assigned classrooms. Responsible for assisting in the training and support of Direct Support Professional/Substitute Staff, under the direction/guidance of the Habilitation Program Coordinator to ensure staff are competent to provide services and meet the needs of people receiving services in keeping with all regulations. Support the goals, objectives, and philosophy of the agency.

Responsibilities

  • Participate in the implementation of goals, services, community outings, etc. with people receiving services in assigned classroom/site.
  • Assist the Habilitation Program Coordinator with the interview process for prospective DSP employees.
  • Recommend and be involved with all remedial assistance for assigned staff.
  • Intervene in instances of insubordination and deficiencies with job performance by verbally conferring with employees and inform the Habilitation Program Coordinator immediately.
  • Schedule and conduct staff meetings at least two times per month to discuss program issues, concerns, review safeguards, prepare for support meetings, and address other topics as outlined on the staff meeting minute format.
  • Assist the Habilitation Program Coordinator with the completion and presentation of staff performance evaluations for staff in assigned site.
  • Complete agency management series.
  • Review progress notes for accuracy and completion; notify the Habilitation Program Coordinator of all discrepancies.
  • Ensure compliance with Medicaid waiver standards, contemporaneous documentation procedures, CQL standards, and all other Arc Jefferson-St. Lawrence policies and procedures.
  • Maintain positive working relationships with all members of the support team.
  • Maintain good family contact, documenting all contacts with family in the Therap system on the family contact ISP program.
  • Report maintenance needs to the Habilitation Program Coordinator, ensuring the maintenance of neat and attractive work areas.
  • Request purchase of necessary program supplies in conjunction with the Habilitation Program Coordinator and Assistant Director of Habilitation Services.
  • Ensure implementation of ISP programs and staff services.
  • Participate with other members of the Support Team in the completion of initial and annual comprehensive functional assessments for People We Support.
  • Administer medications as directed by physicians’ orders and agency policy after successfully completing the OPWDD-approved medication course and maintain certification.
  • Ensure proper storage is available for all classroom equipment and supplies.
  • Complete incident/accident reports as requested.
  • Complete attendance in Therap to ensure ability to bill for services provided.
  • Complete and submit weekly attendance summary search for review by the Habilitation Program Coordinator.
  • Maintain current van certification.
  • Ensure all program staff are aware of pertinent information regarding people receiving services.
  • Demonstrate a working knowledge of all Day Habilitation and agency regulations.
  • Regularly monitor and review all personal funds for People We Support and program money on site as outlined in Day Habilitation procedures; review all ledgers.
  • Participate in quarterly money audits completed by the Accounting Department.
  • Ensure the daily completion of the Day Habilitation daily assignment sheet, with review and approval by the Habilitation Program Coordinator.
  • Complete Fire Drill forms each time a drill is completed and submit to the Assistant Director of Habilitation Services for review and signature.
  • Ensure completion and accuracy of the Vehicle Mileage and Data form each time a vehicle is utilized; submit to Habilitation Program Coordinator at the end of each month.
  • Perform or ensure the completion of the Protecting the Workplace checklist on a daily basis to minimize and address safety hazards and ensure all equipment is being utilized safely and properly.
  • Promote a person-centered philosophy.
  • Perform bus aide duties as assigned, including:
    • Provide assistance and behavioral management to ensure safe transport of all passengers.
    • Assist passengers on and off the vehicle if required.
    • Assist passengers with seat belts when applicable.
    • Monitor behavioral situations during transport in accordance with SCIP-R procedures.
    • Assist driver during an emergency as per established emergency guidelines.
    • Perform a walk-through of the vehicle at completion of each run to ensure all persons and belongings are off the vehicle.
    • When using Agency vehicles to transport people to and from day program or any community activity, ensure vehicle is clean, wheelchair straps are placed in proper area, and the vehicle is in good repair and operational order. Report all concerns to supervisor/administrator and Transportation Department immediately.

The employee frequently is required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds.
