• Develop and implement effective patient care protocols, ensuring seamless transitions from hospital to rehabilitation setting
• Collaborate with our interdisciplinary team to create personalized care plans, addressing the unique needs of each patient
• Serve as a liaison between our facility and external partners, ensuring seamless communication and care coordination
• Provide mentorship and coaching to our nursing staff, empowering them to reach their fullest potential

Requirements

• Bachelor's Degree in Nursing or related field
• Current RN licensure in the state of Missouri
• Minimum of 5 years of experience in a rehabilitation setting, with a focus on long-term care or post-acute care
• Strong leadership and communication skills, with the ability to motivate and inspire our nursing team
• Proven track record of improving patient outcomes and reducing readmissions
• Able to work effectively in a fast-paced environment, with a focus on providing exceptional patient care
