  • EDUCATION: M.D., D.O. or equivalent degree required.
    Completion of an accredited residency required;
    ABPN board certification or board eligibility required.
  • LICENSES: Unrestricted Tennessee medical license at the time of appointment.
    Current DEA certification number,
    Eligible for malpractice insurance
