About the role

Bronson Internal Medicine Portage in Portage, MI is seeking a Nurse Practitioner or Physician Assistant to join their busy established Internal Medicine practice.

• Hospital-employed, outpatient-only position
• Work alongside 5 physicians and 2 advanced practice providers
• Team approach with Ambulatory Care Manager, social worker, pharmacist, and nurse triage
• Minimal call required
• Full-time with flexible scheduling

Responsibilities

• Perform comprehensive and problem-focused histories and physical examinations; interpret laboratory and radiologic tests
• Interpret and correlate subjective and objective patient data to formulate diagnoses
• Under physician direction, review patient findings and develop treatment plans within scope of practice
• Provide care in outpatient and inpatient settings
• Demonstrate competencies specific to the population served
• Coordinate care with the multidisciplinary team to facilitate patient progress toward outcomes, including discharge or transfer of care
• Act as a clinical resource for the multidisciplinary team through assertive coordination, identifying common complications, and integrating quality measures/evidence into practice
• Perform initial comprehensive assessments to determine if patients require higher intensity of service or transfer to hospital setting, applying regulatory and third-party payor criteria
• Facilitate timely discharges/transfers to appropriate level of care
• Evaluate and manage patients, including lab, EKG, and imaging studies
• Complete documentation to support billing, medical care, orders, and progress notes
• Be available for nursing questions and to evaluate patient status changes
• Communicate clearly and effectively verbally and in writing (utilizing SBAR technique)
• Communicate daily with internal customers (
