About the role
Manageable patient panel: Maximum 200 patients
Low daily volume: 10–14 patients per day
Minimal call requirements
Robust interdisciplinary care team including RN case management, health coaches (MAs), pharmacy, social work, care coordination, HCC coding support, and dedicated practice leadership

Responsibilities
Provide personalized, high-quality care to patients 65 years and older in a value-based, Medicare Advantage practice focused on quality, outcomes, and meaningful patient relationships.
Requirements
- Master’s degree in nursing (APRN/FNP) or Physician Assistant Studies
- Must have 2+ years of clinical experience as an APRN or PA-C
Qualifications
- Active Florida APRN or PA-C license
- CURRENT DEA license
- ANCC or AANP Certification
- BLS required; ACLS (preferred)
