APP Float - Primary Care+ Polk/Lake
AdventHealth Primary Care+ is seeking Float Advanced Practice Providers to join our team-based primary care model. About the role
In this role, you will work closely with other Advanced Practice Providers and Care Teams to deliver comprehensive primary care in a modern, well-supported clinic focused on preventive care, chronic disease management, and long-term patient relationships.
Float Regions: Polk County, Lake County

Requirements
- Active Florida APRN or PA-C license
- Primary Care or Family Medicine experience (preferred)
- Strong skills in preventive care and chronic condition management
- Ability to work collaboratively within a team-based care model
- BLS Required
