Mohawk Valley Health System (MVHS) is seeking a Family Medicine Advanced Practice Provider to join our established team of Physicians and Advanced Practice Providers. MVHS is dedicated to supporting our providers to deliver premier healthcare to our region, with patients as our central focus. We operate 17 primary care offices across the Mohawk Valley, offering the option to live in rural or urban communities.

About the role

This full-time position includes a traditional outpatient practice with 35 patient contact hours and 5 hours of administrative time per week. You will work alongside an established, well-supported team.

Responsibilities

  • Perform physical exams on patients from neonate through geriatric populations.
  • Obtain and document patient medical histories; identify health risk factors.
  • Diagnose and treat common acute and chronic illnesses or injuries.
  • Order and interpret diagnostic tests (laboratory work, x-rays, EKG).
  • Prescribe medications and non-pharmacological therapies based on patient diagnoses, health history, and age.
  • Encourage positive health behaviors for disease prevention through education and counseling.
  • Apply standardized care guidelines in clinical practice.
  • Assess and document patient and family psychosocial issues; formulate treatment and education plans.
  • Provide counseling and referrals to other healthcare professionals or community resources as appropriate.
  • Collaborate with physicians and healthcare team members to plan optimal patient care.
  • Respond appropriately in emergent situations.
  • Provide follow-up care, reassess treatment plans, and adjust as needed.
  • Document all findings, interventions, and results.
  • Maintain patient confidentiality and ensure a safe care environment.
  • Provide age-appropriate, culturally sensitive care.
  • Stay current with medical advancements, technology, and new drugs through continuing education.
  • Follow medication safety protocols to reduce errors.
  • Treat patients and families with respect and dignity.
  • Attend departmental, organizational, and medical staff meetings as appropriate.
  • Perform other duties as required.

Requirements

  • Master’s degree from an accredited Nurse Practitioner Program or certification from an accredited NP program or completion of an accredited Physician Assistant Program.
  • Minimum 1 year of experience in a clinical setting.
  • Ability to meet and maintain credentialing requirements and hospital privileges.
  • Knowledge and competency in a wide range of clinical areas.
  • Familiarity with EHR/EMR software.

Qualifications

  • Current NYS Nurse Practitioner license or current NYS Physician Assistant license with prescription privileges.
  • Current BCLS and ACLS certifications.
  • Preferred: Active ANCC certification in relevant specialty.

Pay

$115,000–$125,000 per year.

Schedule

Full-time, days, 8.5-hour shifts, 35 patient contact hours and 5 administrative hours per week.
