Summary Job Summary In collaboration with Maternal-Fetal Medicine (MFM) physicians and under the administrative supervision of the Maternal-Fetal Medicine service leadership, APN-Nurse Practitioner for Diabetes Obstetric provides exceptional care to a diverse patient population with gestational and pregestational diabetes in pregnancy within the high‑risk obstetric service. Promotes wellness and health maintenance by performing comprehensive diagnostic and therapeutic medical services. Provides diabetes education, insulin management, glucose monitoring oversight, and care coordination for patients receiving care in Maternal-Fetal Medicine and high‑risk obstetric clinics. Develops and implements patient management plans in providing continuity of care. Manages diabetes in pregnancy within Maternal‑Fetal Medicine clinics and coordinates postpartum diabetes follow‑up and transition of care to primary care providers.
