Join AmeriCorps as a Health Educator with a focus in Health Promotions at Erie Family Health Centers. This full-time service year opportunity supports health education, chronic disease prevention, patient engagement, and HIV-focused outreach through the Lending Hands for Life program. Main Duties & Responsibilities
- Provide one-on-one health education and self-management support to patients
- Participate in community events, training, and AmeriCorps service activities
- Support Lending Hands for Life HIV programming and re-engagement efforts
- Congduct outreach and engagement for patients requiring additional support
- Document patient interactions and coordinate follow-up care activities
- Facilitate health education groups and recruit participants for programs
Qualifications
- Education Required: High school diploma or equivalent.
- Skills And Knowledge Required: Strong verbal and written communication skills. Ability to work with diverse populations. Current CPR Certification. U.S. citizen, national, or lawful permanent resident. Ability to meet AmeriCorps background check requirements.
- Preferred: Experience in community health, social services, or related fields. Intermediate Microsoft Office skills. Experience with electronic health record systems. Bilingual English/Spanish skills.
