About the role

Join AmeriCorps as a Health Educator with a focus in Health Promotions through Lending Hands for Life at Erie Family Health Centers. This full-time service year opportunity supports health education, chronic disease prevention, patient engagement, and HIV-focused outreach.

Responsibilities

  • Provide one-on-one health education and self-management support to patients
  • Participate in community events, training, and AmeriCorps service activities
  • Support Lending Hands for Life HIV programming and re-engagement efforts
  • Congduct outreach and engagement for patients requiring additional support
  • Document patient interactions and coordinate follow-up care activities
  • Facilitate health education groups and recruit participants for programs

Requirements

  • Education: High school diploma or equivalent
  • Skills and Knowledge: Strong verbal and written communication skills, Ability to work with diverse populations, Current CPR certification, U.S. citizen, national, or lawful permanent resident, Ability to meet AmeriCorps background check requirements

Qualifications

  • Experience: Experience in community health, social services, or related fields, Intermediate Microsoft Office skills, Experience with electronic health record systems, Bilingual English/Spanish skills

Pay
