AmeriCorps Health Educator (Hybrid)
About the role
Join AmeriCorps as a Social Determinants of Health (SDOH) Health Educator at Erie Family Health Centers. This year-long, full-time service position provides the opportunity to help connect individuals and families to resources that support their health and well-being.
Responsibilities
- Screen individuals and families for social drivers of health needs and connect them with appropriate resources.
- Provide referrals and linkage to community services including food, transportation, financial assistance, and other support programs.
- Assist with enrollment in eligible benefits, programs, and services.
- Support outreach activities and follow-up with individuals regarding resource needs.
- Provide digital navigation assistance and help individuals access available services.
- Complete documentation, data entry, and program tracking activities.
Requirements
- Education Required: High school diploma or equivalent.
- Skills And Knowledge Required: Strong verbal and written communication skills, Ability to work with diverse populations, U.S. citizen, national, or lawful permanent resident, Ability to meet AmeriCorps background check requirements.
- Preferred: Experience in community health, social services, or related fields, Intermediate Microsoft Office skills, Experience with electronic health record systems, Bilingual English/Spanish skills.

Pay
