About the Role

The Ambulatory Care RN will perform Medicare Annual Wellness visits in accordance with CMS regulations. This role is responsible for providing quality triage and patient care utilizing strong assessment skills and point of care testing while practicing within the scope of practice defined by the State Board of Nursing. The RN collaborates with clinicians to assess and establish plans of care for complex patients, including setting priorities with the patient and family. The position demonstrates the ability to triage and coordinate patient care with a proactive approach to both acute and chronic disease management while advocating for the patient’s rights, values, and belief systems.

The Ambulatory Care RN works closely with the interdisciplinary care team to improve quality, patient satisfaction, and is involved in the ongoing development and growth of the practice and staff.

Department Scope of Service

The BMH-owned or managed physician practices provide medical services in various specialties and offices, including pediatrics, OB/GYN, cardiology, orthopedics, general surgery, family practice, and internal medicine. The practices are staffed by physicians and other licensed providers supported by clinical staff (RNs, LPNs, or medical assistants) and clerical staff (receptionists, transcriptionists, file clerks). Hours vary by practice.

Responsibilities

  • Perform Medicare Annual Wellness visits in compliance with CMS regulations.
  • Provide quality triage and patient care using strong assessment skills and point of care testing.
  • Collaborate with clinicians to assess and establish plans of care for complex patients, setting priorities with the patient and family.
  • Triage and coordinate patient care with a proactive approach to acute and chronic disease management.
  • Advocate for patient rights, values, and belief systems.
  • Work closely with the interdisciplinary care team to improve quality and patient satisfaction.
  • Contribute to the ongoing development and growth of the practice and staff.

Requirements

  • Active State of Vermont RN license.
  • Current Healthcare Provider CPR certification.
  • Graduate of an RN-accredited nursing program; BSN preferred.
  • 3-5 years of recent outpatient clinical nursing experience preferred.
  • Acute care experience preferred.

This position requires lifting or exerting force:

  • Up to 10 pounds: 2/3 or more of the time.
  • Up to 35 pounds: 1/3 to 1/2 of the time.

Special vision requirements:

  • Close Vision (clear vision at 20 inches or less).
  • Color Vision (ability to identify and distinguish colors).
  • Ability to Adjust Focus (ability to adjust eye to bring an object into sharp focus).

Work Environment

This position requires exposure to the following environmental conditions:

  • Work near moving mechanical parts: Up to 1/3 of the time.
  • Fumes or airborne particles: Up to 1/3 of the time.
  • Toxic or caustic chemicals: Up to 1/3 of the time.
  • Risk of radiation: None.

The typical noise level for the environment is moderate.

Hearing requirements:

  • Ability to hear calls.
  • Ability to hear instructions from physicians, department staff, and others.

OSHA Category

OSHA Category I - Tasks that involve exposure to blood, body fluids, or tissues require the use of Universal Precautions as evidenced by bloodborne pathogens standards.

Pay

Salary range: $39.15–$68.16 per hour. The listed range represents the pay scale reasonably expected for a successful candidate based on factors such as relevant experience, education, internal equity, and market competitiveness. Final compensation will be determined during the hiring process.
