Role Overview
Provides direct patient care in an ambulatory setting, collaborating with providers and the care team to develop and implement care plans that ensure high-quality, cost-effective healthcare outcomes. Supports clinical staff, assists with patient management, and ensures adherence to protocols and compliance standards. Offers patient education through verbal and written instructions, demonstrating critical thinking and problem-solving skills.
Key Responsibilities
- Develops and updates care plans in coordination with PCPs and specialists.
- Assesses physical, psychosocial, and emotional needs of patients using critical nursing skills.
- Ensures clinical protocols and assessment guidelines are followed.
- Supports documentation of patient assessments and treatments in compliance with standards.
- Provides patient education, considering individual learning abilities.
- Works in a safety-sensitive environment, maintaining alertness and safety at all times.
Qualifications & Skills
- Education: Nursing Diploma/Associate's Degree; Bachelor's Degree preferred.
- Experience: 2-5 years required; 5-10 years preferred.
- Licenses & Certifications: RN license, BLS certification.
- Skills: Strong clinical assessment, patient communication, critical thinking, teamwork, and adherence to protocols.
Schedule
- 40-hour day shift schedule
- No call, weekends, or holidays
