About the Role
Provides direct patient care in an ambulatory setting, collaborating with providers and care teams to develop and implement personalized care plans aimed at delivering high-quality, cost-effective healthcare. Supports clinical staff and assists providers with daily patient management, documentation, and assessment.
Responsibilities
- Develop and update care plans in partnership with PCPs and specialists to optimize health outcomes.
- Assess patients’ physical, psychosocial, and emotional needs using critical nursing skills.
- Ensure adherence to clinical protocols, assessment guidelines, and compliance standards.
- Support provider documentation and patient education through verbal and written instructions.
- Provide support in patient management, demonstrating problem-solving and critical thinking.
Requirements
- Nursing Diploma or Associate's Degree; Bachelor's degree preferred.
- 2-5 years of experience; 5-10 years preferred.
- Active RN license and Basic Life Support (BLS) certification.
- Ability to work safely with alertness; strong communication and patient education skills.
