Responsibilities
- Provide medication management, including medication reconciliation and dosing support
- Collaborate with interdisciplinary teams to develop and implement individual care plans
- Monitor medication regimens for therapeutic goals, side effects, and overall effectiveness
- Identify opportunities for drug therapy optimization and cost savings
- Participate in quality improvement efforts and clinical outcome studies
Qualifications
- Doctor of Pharmacy (PharmD) degree
- Active pharmacist license in California
- Advanced Practice Pharmacist credential in California, with required criteria met
- PGY1/PGY2 residency completion preferred, but not required
- Strong verbal communication skills and the ability to analyze information
- In-depth knowledge of pharmacology and FDA regulations

$110,000 – $130,000 annually
