Ambulatory Clinical Pharmacist, Non-Oncology

Responsibilities

• Provide medication management, including medication reconciliation and dosing support
• Collaborate with interdisciplinary teams to develop and implement individual care plans
• Monitor medication regimens for therapeutic goals, side effects, and overall effectiveness
• Identify opportunities for drug therapy optimization and cost savings
• Participate in quality improvement efforts and clinical outcome studies

Qualifications

• Doctor of Pharmacy (PharmD) degree
• Active pharmacist license in California
• Advanced Practice Pharmacist credential in California, with required criteria met
• PGY1/PGY2 residency completion preferred, but not required
• Strong verbal communication skills and the ability to analyze information
• In-depth knowledge of pharmacology and FDA regulations

Pay

$110,000 – $130,000 annually
