Ambulatory Clinical Pharmacist

Contract

Provides ambulatory pharmaceutical care services within the CNEMG offices and Integra ACO and reports to the Director of Ambulatory and Value Based Pharmacy Practice.

Responsibilities

  • Functions as the medication expert on the interdisciplinary care team
  • Reconciles and assesses medication regimens for adherence and optimal therapy to achieve desired outcomes
  • Provides evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary care team
  • Provides medication and disease state management education to patients
  • Practices within established collaborative practice agreement(s) consistent with services delineated within the agreement
  • Participates in the development and/or implementation of quality improvement initiatives
  • Collaborates with providers and patients to ensure high quality, cost-effective care
  • Documents all encounters within the electronic health record(s) as well as other platforms as necessary
  • Communicates appropriately with interdisciplinary care team members
  • Contributes to achievement of contractual obligations for the ACO including but not limited to comprehensive medication reviews, adherence outreaches, targeted medication conversion initiatives, and gaps in care
  • Actively participates in Pharmacy Committee
  • Develops presentations on quality metric data and pertinent clinical topics and delivers to providers
  • Participates in development of new programs, services, and best practices for pharmacy and medical team
  • Precepts and actively contributes to the development of pharmacy students
  • Assists in the development and maintenance of the PGY1 and PGY2 residency program and pharmacy residents
  • Actively participates in care retention initiatives
  • Assists with precepting pharmacy students

Requirements

  • Doctor of Pharmacy (PharmD) degree with 2 years of professional experience or Bachelors of Science in Pharmacy with 3 years experience
  • Current RI Pharmacist license is required
  • PGY-1 Residency or Board of Pharmaceutical Specialties certification preferred
  • Medication Therapy Management certificate required within 6 months of hire
