Ambulatory Clinical Pharmacist
About the Role
The Ambulatory Clinical Pharmacist provides patient care and advanced clinical pharmacy services in the ambulatory pharmacy space, including referral-based HOPD pharmacist-managed clinics, specialty pharmacy, and population health pharmacy services. This role involves collaborating with prescribers in the ambulatory setting and providing direct patient care to manage medication therapy.
Responsibilities
- Deliver advanced clinical pharmacy services in ambulatory settings.
- Collaborate with prescribers to manage medication therapy for patients.
- Ensure effective communication with prescribers regarding medication-related problems and potential solutions.
- Document patient interactions across various electronic medical record (EMR) platforms.
Requirements
- Current state Registered Pharmacist License.
- B.S. in Pharmacy or PharmD degree.
- PGY-1 Pharmacy Practice Residency (significant experience may be considered in lieu of residency).
- Board certification (BCPS, BCACP, or equivalent) within 18 months of hire.
- Strong verbal and written communication skills.
- Strong customer service skills.
Preferred Qualifications
- PGY-2 residency in ambulatory care or a related specialty.
- Expertise in 340B program operations and compliance.
- Proficiency in EPIC or similar EMR systems.
- Advanced clinical knowledge in specialty areas such as diabetes, cardiology, pulmonology, rheumatology, or oncology.
- Experience as a pharmacy residency preceptor in ambulatory pharmacy services.
Pay
A reasonable compensation estimate for this role is $115,107.20 to $166,712.00. The exact starting compensation will be determined based on factors such as skillset, years of applicable experience, education, credentials, and licensure.
