Ambulatory Clinical Pharmacist

Department: Ambulatory Pharmacy • Per Diem (as needed) for weekday shifts • Prime Healthcare St Francis Hospital, Evanston

Responsibilities

• Provide patient care and advanced clinical pharmacy services in the ambulatory pharmacy space, including referral-based HOPD pharmacist-managed clinics, specialty pharmacy, and population health pharmacy services.
• Collaborate with prescribers in the ambulatory setting and provide direct patient care managing medication therapy.
• Ensure effective communication with prescribers regarding identified medication-related problems and potential solutions.
• Document patient interactions across various electronic medical record (EMR) platforms.

Qualifications

Required Qualifications

• Current state Registered Pharmacist License.
• B.S. in Pharmacy or PharmD degree.
• PGY-1 Pharmacy Practice Residency required (significant experience may be considered in lieu of residency).
• Board certification (BCPS, BCACP, or equivalent) within 18 months of hire.
• Strong verbal and written communication skills.
• Strong customer service skills.

Preferred Qualifications

• PGY-2 residency in ambulatory care or a related specialty.
• Expertise in 340B program operations and compliance.
• Proficiency in EPIC or similar EMR systems.
• Advanced clinical knowledge in specialty areas such as diabetes, cardiology, pulmonology, rheumatology, or oncology.
• Experience as a pharmacy residency preceptor in ambulatory pharmacy services.
• Specific ambulatory experience in managing chronic diseases.

Pay

Reasonable compensation estimate for this role is $70.84 per hour. The exact starting compensation will be determined based on skillset, years of applicable experience, education, credentials, and licensure.
