Description

An independent clinic-based pharmacist practice with broad scope of practice
Delivering comprehensive support to providers and patients by answering drug information inquiries, resolving pharmacy-related issues, and providing medication education
Working as part of a multidisciplinary team with physicians, advanced practice practitioners, behavioral health specialists, and care managers
Collaboration with clinical pharmacy technicians
Opportunities for precepting pharmacy students and residents

Requirements

• Part Time; Monday – Friday (either 2.5 days per week or rotating 2 or 3 days per week every 2 weeks): 8 am to 5 pm
• Preferred Qualifications: Previous experience with the clinical management of primary care based chronic diseases
  Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience
  Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)

Qualifications

Pharmacists in this role independently manage chronic disease states while working closely with primary care providers.
