Description
An independent clinic-based pharmacist practice with broad scope of practice
Delivering comprehensive support to providers and patients by answering drug information inquiries, resolving pharmacy-related issues, and providing medication education
Working as part of a multidisciplinary team with physicians, advanced practice practitioners, behavioral health specialists, and care managers
Collaboration with clinical pharmacy technicians
Opportunities for precepting pharmacy students and residents
Requirements
- Part Time; Monday – Friday (either 2.5 days per week or rotating 2 or 3 days per week every 2 weeks): 8 am to 5 pm
- Preferred Qualifications: Previous experience with the clinical management of primary care based chronic diseases
Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience
Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
Qualifications
