About the role
Provide specialized patient care in a growing Heart Failure Clinic, focusing on patient education, triage, IV starts for CHF patients, and procedure scheduling.
Responsibilities
- Collaborate with healthcare providers to develop and implement care plans that promote wellness and optimize outcomes.
- Assess and evaluate patients’ physical, psychosocial, and emotional needs using critical nursing skills.
- Ensure clinical protocols and assessment guidelines are followed in compliance with standards.
- Support documentation of patient assessments and treatments according to compliance standards.
- Educate and support patients and families, utilizing appropriate communication tools and techniques.
- Maintain safety awareness, working in a constant state of alertness.
Requirements
- Nursing Diploma/Associate's Degree in Nursing required; Bachelor's preferred.
- 2-5 years of nursing experience; 5-10 years preferred for advanced roles.
- Current RN license and BLS certification required.
- Experience in cardiology or heart failure patient care is preferred.
Schedule
Monday-Friday, typically 7:00 AM-3:30 PM, with no weekends.
