Aging Case Aide 2 - Adult Daily Living Center - Hawley (Local Government)

About the role

This position supports older adults by providing information, guidance, and help with important programs and activities across Wayne County. You will split your time between Hawley, PA and Honesdale, PA.

Responsibilities

• Benefits Guidance: Assist older adults in understanding programs such as Medicare, PA MEDI, PACE, LIHEAP, and Tax or Rent Rebate
• Application Support: Help clients complete applications and connect with community resources
• Community Outreach: Conduct outreach across Wayne County, including home visits to homebound individuals
• Program Coordination: Organize senior center activities, support volunteers, and manage registration and assessments
• Nutrition Assistance: Support meal distribution, sanitation steps, and donation documentation
• Record Management: Maintain accurate records and complete data entry in agency and statewide databases

Schedule

• Full-time employment
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with 30-minute lunch
• May require overnight travel or out-of-county travel for meetings or training
• May cover at other senior centers or satellite locations
• No telework option available

Requirements

• Minimum Experience and Training Requirements:
  • One year as an Aging Case Aide 1; or
  • Two years of public contact work, including one year in a human services program; or
  • Any equivalent combination of experience and training
• Must meet the PA residency requirement
• Must be able to perform essential job functions
• Must pass a background investigation
• Conditional offer of employment requires tuberculosis and drug screening
• Position falls under the provisions of the Child Protective Services Law
• Conditional offer requires submission and approval of satisfactory criminal history reports including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance
