About the role
Do you love helping children and families thrive? TFI Family Services is hiring a full-time Aftercare Case Manager to join our team!
As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll focus on risk management and help families maintain stability during the aftercare period.
Responsibilities
- Provide crisis intervention and work with public and private agencies offering supportive services.
- Maintain accurate documentation and reports.
- Coordinate community and educational services for families.
- Meet with families in-person to support reunification and resolve any concerns.
Requirements
- A bachelor’s degree in human services.
- A valid driver’s license.
- A willingness to learn—we’ll provide all the training you need!
