Average hourly rate is $26.43 (including wage, incentives, bonuses, overtime, shift differential, etc.).

About the role

TFI Family Services is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll focus on risk management and help families maintain stability during the aftercare period.

Responsibilities

• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.

Requirements

• A bachelor’s degree in human services.
• A valid driver’s license.
• A willingness to learn—we’ll provide all the training you need!
