Hospice LVN – After-Hours / Weekend Position Summary
The Hospice After-Hours/Weekend LVN provides skilled nursing care, patient support, and compassionate end-of-life care to hospice patients and families during evenings, nights, weekends, and holidays. This role supports continuity of care outside of regular business hours by responding to patient and caregiver needs, assisting with symptom management, providing education, completing visits as needed, and collaborating with the RN Case Manager, hospice physician, and interdisciplinary team.
Key Responsibilities
- Provide after-hours, weekend, evening, overnight, and holiday nursing support to hospice patients and families as scheduled.
- Respond promptly to patient and caregiver calls, assisting with triage needs by phone and making in-person visits as appropriate.
- Provide skilled nursing visits in patient homes, facilities, or other care settings under the direction of the RN.
- Absorb and support hospice admissions as directed by the RN, including gathering information, supporting documentation, obtaining signatures as appropriate, and helping coordinate patient and family needs.
- Perform focused nursing assessments and report changes in patient condition to the RN Case Manager, on-call RN, hospice physician, or clinical leadership as appropriate.
- Maintain patient comfort related to pain, shortness of breath, anxiety, nausea, agitation, and other symptoms.
- Administer medications, treatments, and comfort measures as ordered and within LVN scope of practice.
- Educate patients and caregivers on medications, comfort measures, disease progression, hospice services, and when to contact the hospice team.
- Communicate changes in patient condition, urgent needs, or concerns to the RN and interdisciplinary team in a timely manner.
- Collaborate with physicians, hospice medical directors, RN Case Managers, social workers, chaplains, aides, and other team members to support the patient’s plan of care.
- Accurately document all visits, patient care, interventions, communication, and changes in condition in the electronic medical record in a timely manner.
- Provide emotional support to patients and families, including during active dying and post-death care.
- Absorb and assist with post-death procedures and required communication in accordance with company policy and state regulations.
- Ensure compliance with hospice policies, procedures, documentation standards, and regulatory requirements.
- Participate in handoff communication with RN Case Managers, weekday clinical staff, and leadership to ensure continuity of care.

- After-hours shifts, weekends, evenings, overnight, and/or holidays as scheduled.
- On-call availability may be required.
- Flexible scheduling based on organizational needs.
