Position Highlights
Position: Advanced Practice Provider (APN/PA) - Post Acute Care
Location: Skilled Nursing Facilities in Endeavors north region (Evanston, Swedish Hospital)
Full Time/Part Time: Full-Time, 40 hours per week
Hours: Monday - Friday between 8:00 a.m. and 5:00 p.m.
Potential for weekend hours in the future
No evenings or holidays
What You Will Need
- License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
- Education: Completion of Master's level program in specialty area
- Certification: BLS/CPR certification required
- Experience: Minimum of three years advanced practice provider experience. Experience with advanced directive and goals of care.
What You Will Do
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
- Promote wellness and prevent illness and injury in various settings for individuals, families, and communities
- Work autonomously and in interdisciplinary teams as resources and consultants
- Positively contributing to safety outcomes and promoting high quality patient experience
