Advanced Practice Provider - Family Medicine Care, Stop 11
About the role
Community Health Network is seeking an Advanced Practice Provider to join their family medicine practice at Stop 11, Family Medicine Care, located at 5908 E. Stop 11 Road, Indianapolis, IN 46237.
Responsibilities
- Provide comprehensive primary care services to patients of all ages within the family medicine practice.
- Utilize a single, robust electronic medical record system (EPIC).
Requirements
- Family Medicine experience as a Nurse Practitioner is preferred.
- Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification.
- Maintain certification on an ongoing basis.
- Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing.
- Maintain eligibility for prescriptive authority on an ongoing basis.
Qualifications
- Graduate from an accredited program in advanced practice nursing.
- Pass a national certification exam relevant to advanced practice nursing.
- Current national certification.
- Eligible for prescriptive authority.
