Advanced Practice Provider - Family Medicine Care, Elwood

About the role

Community Health Network is seeking an Advanced Practice Provider to join their integrated healthcare system with over 1,300 providers in Indianapolis, IN. The position is located at Family Medicine Care, Elwood, IN 46036.

Responsibilities

• Work in a single, robust electronic medical record system (EPIC)
• Participate in outstanding provider engagement and patient satisfaction activities
• Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification
• Maintain certification on an ongoing basis
• Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing
• Maintain eligibility for prescriptive authority on an ongoing basis

Requirements

• Minimum of 3–5 years of experience as an Advanced Practice Provider in a primary care setting
• Must hold a valid license to practice as an Advanced Practice Provider in the state of Indiana

Qualifications

• Family medicine training
• Nationally recognized Advanced Practice Nurse certification
• Eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing

• Ability to work collaboratively with other healthcare professionals
