About the role

The Family Medicine Advanced Practice Provider role is located in Phoenix, Arizona at Dignity Health Medical Group. This position offers a unique combination of clinic-based and virtual care services.

Responsibilities

  • Provide comprehensive primary care to patients of all ages, including preventative care, acute and chronic disease management, and health education
  • Conduct thorough physical examinations, order and interpret diagnostic tests, and develop individualized treatment plans
  • Prescribe medications and treatments in accordance with state regulations and practice protocols
  • Perform minor in-office procedures (e.g., Derm/MSK procedures and Nexplanon)
  • Maintain accurate and timely electronic health records (EHR)
  • Collaborate effectively with physicians, nurses, MAs, and other healthcare professionals
  • Participate in team meetings, quality improvement initiatives, and continuing education
  • Provide virtual primary care consultations via secure telehealth platforms
  • Assess, diagnose, and treat a wide range of acute and chronic conditions amenable to virtual care
  • Order necessary labs, imaging, and prescriptions remotely
  • Provide appropriate referrals to specialists or in-person care when clinically indicated
  • Educate patients on their health conditions, treatment plans, and self-management strategies
  • Adhere to all telehealth best practices, privacy regulations (HIPAA), and state licensing requirements for virtual care
  • Utilize virtual care tools and technology proficiently

Requirements

  • Current and unrestricted licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Arizona
  • Current national board certification (ANCC, AANP, NCCPA)
  • BLS certification
  • Minimum of 3 years of experience as an Advanced Practice Provider in Family Medicine or Primary Care

Qualifications

  • Proficiency with Electronic Health Records (EHR) systems; experience with Cerner is a plus
  • Tech-savvy with the ability to quickly learn and adapt to new technologies
