Family Medicine Advanced Practice Provider

Provide comprehensive outpatient family medicine care across all age groups at a primary care clinic. Work collaboratively with physician support to deliver high-quality patient care in a hospital-employed practice setting. Utilize Epic electronic health record system to document and manage patient information efficiently. Manage an average of 18–22 patients per day, ensuring thorough assessments, diagnoses, treatment plans, and follow-up care. Key requirements include strong clinical skills in family medicine, excellent communication abilities, and the capacity to work effectively within a team. Responsibilities encompass providing preventive, acute, and chronic care, maintaining accurate documentation, and adhering to best practices and healthcare standards.
